  • COMPLETE NEW PARTICIPANT PACKET

 The New Participant Packet contains an Application, Consent &   Release Form and Medical History Form (which needs to be             thoroughly filled out, signed by an MD, DO, NP or PA and dated to   be valid).



  • SUBMIT NEW PARTICIPANT PACKET

  This can be done in one of the following ways…

  1.   Email to cheerfulheartstherapy@gmail.com
  2.   Mail to CHAT (7317 Avenue P, Santa Fe, TX 77510)


  • EVALUATION

  Once CHAT has received your completed application, we will       contact you to schedule an evaluation. An evaluation helps us   ensure that we can safely accommodate you in our program,   recommend the proper class, develop therapeutic goals and create   good rider/volunteer/horse teams. Not everyone can be accepted   into the program due to safety or physical / medical   contraindications.


  • REGISTER for APPROPRIATE/DESIRED PROGRAM

  Upon completion of an evaluation and determination by CHAT that   we are able to safely accommodate the participant, the participant is   able to register for services. A participant’s individual needs and   abilities determine which service is best suited for registration.

BASIC INFORMATION ABOUT SERVICES


  • Group classes will be available as we grow
  • Class cost is dependent on type of services provided and length of class
  • Services are provided year round on a session by session basis.  Sessions generally last six weeks with a one to two week break at the end of each session.
  •  Classes are held Tuesday-Saturday with varying times.
  • Clients must make a commitment for the full length of the session.  Clients may pay week to week or for the whole session at once.  All clients will pay for the full session regardless of cancellation.  We will do everything in our power to provide a makeup class, but there is no guarantee.
  • We do not bill directly to health care providers, but we will gladly provide paperwork and receipts, if asked, so a client can seek reimbursement.
